Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years in Acute Care Case Management/Discharge Planning License: Valid California state license required; pending license accepted if licensed upon start date Certifications: None specified Must-Have: - Must have hospital experience - Behavioral health experience does not apply - Weekend coverage required to meet department needs (every other weekend) Description: The registered nurse will work as a case manager. Experience is required in an acute care setting. Travelers may be expected to float between nearby sister facilities. Travelers' refusal to float may result in termination of assignment. The nurse is expected to commit to working 3 of the designated holidays. All requests for time off or specific scheduling must be made upfront during the application process. Post-application requests may not be considered. Inflexibility regarding the contracted schedule may lead to contract termination. Two references covering one year of experience within the last three years are required. This role requires a comprehensive understanding of discharge planning processes. The nurse will work with patients across various age groups. Patient-to-nurse ratios are 1:25 during the day and 1:45 on weekends. Knowledge of hospital charting systems is expected.
